Thank you for your interest in serving as a volunteer at Cottonwood Creek. We value your willingness to be a part of our team!

There are 2 steps to this process: our doctrinal beliefs section as well as the actual background check. For the beliefs section, please type in your name, answer each question and hit the “submit” button at the bottom of the page. Once you’ve submitted the beliefs statement you will then be directed to the Volunteer Application Form.

On the Volunteer Application form, please fill out the first page as completely as possible to help ensure an accurate match with state/national records. When completed, please click on the “SEND” button which will take to you a page where you will have the opportunity to identify ministry for which you are intending to volunteer. (ex: Children’s/Sports/AWANAs/etc…) And finally, click on the “SEND” button again to access the final page, which is a consent form to run the background check. The entire process should take between 5-10 minutes to complete.

Must be 18 or older to complete a background check.

Questionnaire & Background Check

This form is designed to help you understand what Cottonwood Creek Baptist Church believes to be true on certain doctrinal issues. Please indicate whether you agree or disagree with each of these statements of belief. Your answers may not necessarily impact your opportunity to serve at Cottonwood Creek Baptist Church.

Why are you filling out this form?*

Which Campus are you filling out this form for?*

If you are filling out this form in order to volunteer, please select all of the ministries you would like to volunteer with:

I believe in the one eternal God, Creator and Ruler of all things, revealed in Three Persons: The Father, Son and Holy Spirit.

To the statement above:*

I believe in Jesus Christ, who is co-equal with the Father and the Holy Spirit. He came down from heaven, took on flesh, was born of a virgin, lived a perfect life, was crucified, rose again on the third day, ascended into heaven, and sits at the right hand of the Father.

To the statement above:*

I believe in the Holy Spirit, who is co-equal with the Father and the Son. He is the Agent of conviction, the Giver of new life, the One who fills up and guides every believer and the Church to accomplish the work of God, and the One who seals all believers until the day of redemption.

To the statement above:*

I believe humanity is created in the image of God and since the disobedience of Adam and Eve, all are sinners separated from God and in need of His salvation.

To the statement above:*

I believe salvation is the free gift of God’s grace found only in Jesus Christ, and if you confess that He is Lord and believe God raised Him from the dead, you are saved.

To the statement above:*

I believe Jesus Christ will come again and judge the living and the dead, that those who have trusted in Christ as their Lord and Savior will spend eternity in His presence, and those who have rejected Christ will be eternally separated from Him.

To the statement above:*

I believe the Bible is breathed-out by God and is inerrant in its original form. I believe that every Christian is called to be actively engaged in a local church and pursue a life that models a love for God and a love for others.

To the statement above:*

If you marked "unsure" on any questions above, would you like to have a Cottonwood Creek Staff member contact you to help answer questions you may have about why the church believes the way they do about the doctrinal issues?

If you marked disagree to any of the above statements, will you agree to support the church's beliefs on the doctrinal issue if it comes up in a teaching or public situation?
