1. Faith Questionnaire (and Children’s Ministry/Student Ministry Policies, if applicable): Below, please type in your name and answer each question
2. Anti-Harrasment Guidelines: Cottonwood Creek Church is committed to a zero-tolerance policy for any type of harassment, ensuring the safety and dignity of its members, guests, volunteers and staff. Prompt and appropriate action will be taken against any form of harassment that is reported to a staff member of the church.
3. Submittal of Faith Questionnaire and Anti-Harrassment Guideline: When you are done answering each question below and hit the “Submit” button at the bottom of the page, you will be redirected to the Background Check form.
4. The Background Check Form:  On the Background Check form, please fill out the first page as completely as possible to help ensure an accurate match with state/national records. When completed, please click on the “SEND” button which will take to you a page where you will have the opportunity to identify ministry for which you are intending to volunteer. (ex: Children’s/Sports/AWANAs/etc…) And finally, click on the “SEND” button again to access the final page, which is a consent form to run the background check. The entire process should take between 5-10 minutes to complete.

Must be 18 or older to complete a background check. If you are under 18, you can contact the ministry you want to serve with and they can get you plugged in to serve with an adult.
